Ocmin Health Care is a Minnesota 245D licensed provider based in Brooklyn Park. We support children and adults with disabilities to stay safe, grow their skills and take part in the life of their community.
Every person we serve has a plan built with them, their family and their case manager. Our staff follow that plan every day and tell you how things are going.
