- Contact and ride request forms: your name, phone number, email address, the reason you are contacting us, an appointment date if you give one, and your message.
- Employee Application: the details a new employee enters on the application, such as name, date of birth, address and contact details, and information about the client they will support.
- Phone calls and emails you send us directly.
Please do not send medical details through the website forms. We will talk about those with you by phone.
How we use it
- To answer your questions and call you back.
- To schedule and confirm rides.
- To start services, working with you, your family and your case manager.
- To process employee applications and meet our hiring and licensing requirements.
We do not sell your information or use it for advertising.
Who can see it
Only Ocmin Health Care staff who need it to help you. We share information with others only when you ask us to, when it is needed to provide your services (for example, with your case manager or the program paying for your rides), or when the law requires it.
How we protect it
The website uses a secure connection. Employee Applications are password protected and stored encrypted, and only signed-in HR staff can open them. We keep information only as long as we need it for the reasons above or as Minnesota and federal law require.

We use cookies only to keep HR staff and job applicants signed in. We do not use advertising or tracking cookies.
